IRS Form 1095-C

You will be receiving IRS Form 1095-C before March 31, 2016. This form is required by the Affordable Care Act (ACA) to report qualifying health coverage offered to you through Montefiore or a Collective Bargaining Agreement.

You do not need to file this form with your tax return but you may need to refer to the information on your 1095-C to complete your tax return.

If you know that you and your eligible dependents had health coverage for all 12 months during 2015, just check the “full year coverage” box on your tax return.
